Welcome to the Pharmacy Benefits Management (PBM) Clinical Pharmacy Practice Office (CPPO)

For nearly 45 years, Clinical Pharmacists and Clinical Pharmacist Practitioners (CPP) have demonstrated their impact on improving medication safety, quality of care and clinical outcomes for Veterans as an advanced practice provider (APP). PBM CPPO’s vision is to transform VA clinical pharmacy practice to provide the highest standard of care for our Veterans and position the Veterans Health Administration (VHA) as a model of clinical pharmacy practice for diffusion and innovation throughout the U.S. Healthcare System.  CPPO is committed to optimizing VA clinical pharmacy practice across all care settings to improve access and provide safe quality care in a cost-effective manner while maintaining Veteran experience as a top priority. This will be accomplished by ensuring all Veterans have access to comprehensive medication management services provided by Clinical Pharmacist Practitioners (CPP) and developing strategies to empower the pharmacy workforce to adapt to changing needs of our Veterans as part of a high reliability organization.

The CPP is a core team member that improves access and quality through the provision of comprehensive medication management (CMM) services.  The CPP is a clinical pharmacist with a scope of practice or privileges authorized by the medical staff and within the medical staff bylaws as an Advanced Practice Provider (APP).  The CPP provides direct patient care and functions at the highest level of clinical practice, working with autonomy and independent decision-making within the parameters of their scope of practice or privileges. At the forefront of practice, VA CPP continue to innovate and show their dedication to the VA mission of caring for Veterans through integration into interprofessional teams, addressing VA facility patient gaps in care, and providing CMM services across the continuum of care. We hope that sharing key information on the practice of the VA clinical pharmacy will promote expansion of the CPP role and ensure patients receive the expertise of this valued team member.
